File a PhotoCare or PhotoCare Plus claim
If your equipment was lost, stolen or damaged, please follow these steps:

PhotoCare claims packet
Complete the checklist within the packet
• Don’t forget to fill out the Statement of Loss, Damage and Repairs form.
Note: If you would prefer, you can fax your claim packet to 913.652.3973 or mail to the attention of PPA Claims at P.O. Box 410679, Kansas City, MO 64141-0679, though these methods may take longer than email.
